Too Low
Problem
You lose distance and control because your takeaway is too low to the ground or you are trying too hard to create power.
Cure
When your swing is too low to the ground, you delay the proper hinging of your wrists until it is too late in your backswing. If you focus too much on creating power, it will cause your arms to swing out too far during the backswing, leading to poor posture and causing a reverse pivot. You will need a proper wrist hinge during your backswing to help eliminate the need to swing the arms too far at the top.
Therapy
Allow your forward arm and the clubshaft to form a forty five degree angle. When you do this it will cause your wrists to hinge halfway to ninety degrees. Your hands should stay close to the ground while you move the club head up quickly. Try to point the thumb on your forward hand towards your back shoulder as soon as possible. Your forward arm should be parallel to the ground while the club shaft is perpendicular to it. To help improve constancy, grip a wedge so your hands are ahead of the club shaft. Now swing the club back about waist high. Focus on keeping your wrists set at impact. When you are using short irons, focus on cocking your wrists as you take the club back and turn through the shot. When you are using longer irons, remember that you need to add fuller shoulder turn.
One thing I've found that helps this is to push down on the grip end of the club with the left hand during the backswing to help get the club into the air. This also helps get a left side feeling into the swing.
