The Flubbed Pitch
Problem
You are flubbing your pitches because you are either slowing down during your downswing or the ball is placed too far forward in your stance.
Cure
If the ball is placed too far ahead, it will cause the club to bottom out before you hit the ball and cause a fat shot with very little power. You can try moving the ball back an inch or two in your stance to see it this fixes the problem. If you are slowing your downswing down instead of accelerating it, your hands will move faster than your body and cause a very steep swing.
Therapy
Your forward swing and backswing should mirror one another; meaning, how far you bring the club back should be equal to how far forward you should swing it during your follow-through. Put most of your weight onto your forward foot at address. Take the club back slowly while you focus on turning your shoulders and arms at the same time. At the top of your swing, you need to begin turning your body towards the target with relaxed arms. You should start increasing in speed due to the club weight and gravity as your arms drop into the downswing. Your weight should shift towards the target as you make contact with the ball.
