Problem
You tend to scoop the ball because you lean the shaft forward and place the ball too far back in your stance, thereby reducing loft. You need to improve your pitching to take strokes off of your game.
Cure
A good pitch shot is when you use more of your arms during your backswing and more of a body rotation during the forward swing.
Therapy
Take an open stance with the ball a little forward towards your front heel. You should place most of your weight onto your forward foot. To prevent scooping, the club shaft should be nearly vertical at the top of your swing so the grip of the club is pointing towards the ground. As you take your forward swing, rotate your hips, shoulders and legs so your body points left of the target, for right handed players. A great drill to help you feel the importance of placing the weight onto your forward foot during your swing is to put a golf ball under the heel of your back foot. This will place most of your weight on your forward foot as your back foot is on the toes. Notice how the club goes back steeper, the face stays open at contact, and your weight starts and finishes on your forward foot.
