On the Mark
Problem
You keep picking your head up before you have completed your putting stroke.
Cure
If trying to listen to the ball roll into the cup does not work for you then try a drill that will train your mind to keep looking at the original ball position.
Therapy
Practice putting with something under the ball such as a flat ball marker, washer, or a coin. As you putt, think of it as a reward to see and stare at the marker. If this marker is good to putt off of then try to keep it with you every time you practice, as a special marker. When the ball is in play, pretend that the marker is still there. Because you miss it, keep your eyes focused on where it should be.
