Off the Toe and Heel
Problem
You pendulum putt a lot off of the toe and heel of your putter.
Cure
Practice putting while trying to keep the clubface square to the target. Cross handed putting is great for your set up position. It squares your shoulders up better and it gets them on a level of plane so it helps you rock better through your putts.
Therapy
Use a training aid based on your putting style. For a simple pendulum stroke, you can practice putting between two clubshafts without touching the sides of the shaft. Set your putter on the ground and place the two clubshafts a quarter inch away on either side of the putterhead. You might want to prop the clubshafts up a little. As you practice your pendulum stroke, work on gaining an even pace without touching the shafts. You might find it helpful to count as you do this. You will be forced to keep the putterhead square to the target as you do this because if you go off your line, you will hit a shaft.
