Check Your Thumbs
Problem
The clubface is not squaring at impact and it is not from the lack of forearm rotation.
Cure
You might be rotating your forearms incorrectly or too much during the backswing. To check to see if this is the problem, you should look to see what direction your thumbs are pointing during the backswing.
Therapy
Practice swinging without a club to check to see where your thumbs are pointing. Stop your swing when your arms are almost shoulder high. At this point, your thumbs should be pointing up, towards the sky. If they are not, start the swing over and keep practicing a proper takeaway until they are. Swing through, into the follow through and make sure that your thumbs are again pointing towards the sky at this level. Repeat this drill with the club in your hands a few times before hitting a bucket of golf balls.
