Arm Tension
Problem
Your upper body is too rigid. This causes you to lose distance as well as accuracy.
Cure
Instead of focusing on trying to reduce the tension in your arms, focus more on the feel of the club as you swing. If your arms are too tense to truly feel the club you will be too likely to steer the club or fail to make proper contact with the ball. Flick your wrist up and down a few times before addressing the ball to get a better feeling the weight of the clubhead.
Therapy
Modify your grip during your practice swing to allow you to have a better feel for the club. Grip the club as usual with your hands. Then with your trailing hand, move your index finger on the top side of the grip. Allow your index finger and thumb to press together forming a circle. Now you should have only the last three fingers of your trailing hand griping the club. Swing the club as usual and you should notice a little less tension and better feel. Try to establish this same feeling when you set up to the ball without the grip modification.
